RETINAL HEMORRHAGE POST BONE MARROW TRANSPLANTATION TO TREAT HODGKIN LYMPHOMA

Objetivo

To report a retinal hemorrhage post bone marrow transplantation to treat Hodgkin lymphoma.

Relato do Caso

A 24-year-old caucasian man complain of sudden visual loss in the left eye (OS). The patient has Hodgkin lymphoma and underwent bone marrow transplantation 2 months ago. On examination, the best corrected visual acuity (BCVA) was 20/25 in the right eye (OD) and 20/300 in the left eye (OS). Anterior segment was unremarkble on both eyes (OU). Intraocular pressure was normal. Fundoscopy revealed preretinal and intrarretinal hemorrhage on the foveal region associated with a roth spot in the lower temporal arcade in the OS. OD was normal. Macular OCT showed subhyaloid and intrarretinal hemorrhage in the OS in the foveal region (Figure 1). YAG laser hyaloidectomy was attemped without sucess. Patient returned after 2 weeks with improvement of visual acuity (20/90) and of the retinal hemorrhage (Figure 2).

Conclusão

Ocular complications following allogenic hematopoietic cell transplantation are usually confined to the anterior segment, but severe posterior segment complications can also occur. Retinal hemorrhage is the most common posterior segment complication and usually occur as a consequence of other pathologies such as CMV retinitis and neovascularization due to ischemid retinophaty. We report a case of retinal hemorrhage post bone marrow transplantations with spontaneous improvement.
